Job summary

An exciting opportunity has arisen for a Band 6 Mental Health CNS or Mental Health Occupational therapist to join Hepatology / Liver Transplant services working in the dedicated liver well-being team based at St Jamess Hospital in Leeds.

The successful candidate will be an enthusiastic, dedicated and innovative. The successful candidate must have experience in mental health, assessing and supporting people who use or have used alcohol or other substances in a harmful way. A good understanding of the impact of poor physical health on psychological functioning is not essential as support, guidance and training will be given from the Band 7 CNS and the team of Hepatology consultants

The role involves assessing and identifying individual patients mental health needs under the care of the Hepatology / Liver transplant service. The role also includes liaising closely with the multi-disciplinary teams and with primary care and referring centres.

We are a small friendly team and welcome a driven individual to join us with new ideas to improve patient pathways and drive forward the expansion of this service whilst becoming highly specialised in this dynamic and highly specialised field.

Main duties of the job

The post holder will have a key role in the development and delivery of a specialised services for hepatology and transplantation across LTHT. They will undertake well-being assessments on patients, including those undergoing liver transplant assessment.

The post holder will be responsible for managing their own caseload of patients they will identify any needs of the patient, with regard to their mental health, alcohol history, social situation and/or general well-being.

They will plan and implementing on-going support in conjunction with other multi-disciplinary team members.

The post holder will be responsible for ensuring effective interdisciplinary and multidisciplinary partnership working with key services.

The post holder will ensure patients are managed in accordance with local and national standards and guidelines.

The AMS CSU brings together a number of specialist services to create one of the largest and most exciting Clinical Services Units within the Leeds Teaching Hospitals NHS Trust.

The Liver Well-Being Team is a multi-disciplinary team consisting of an alcohol specialist, social worker and specialist advice worker. We offer support to liver patients with regard to housing, finances, mental health, substance use and general well-being. We work closely within the Hepatology and Liver Transplant service.
