JOB
PURPOSE/SUMMARY
The
Jobholder is responsible for the management of a defined caseload. The
jobholder carries continuing responsibility for the assessment of care needs,
the development, implementation and evaluation of programmes of care and the
setting of standards of care. The jobholder will provide specialist assessment
care and advice to a specific patient group and will contribute to the
development of the speciality service. The jobholder will ensure cost
effective use of resources.
Current
services comprise providing specialist advice support and training to ensure high
quality discharge planning is undertaken by wards staff.

4. PRINCIPAL DUTIES & AREAS OF
RESPONSIBILITY
Fulfils
the requirements as directed by the Nursing and Midwifery Council or Health
Care Professions Council to maintain and revalidate registration
Clinical
Clinical
care delivery
- Provide a clinical lead and role
model to clinical staff by delivering high quality care.
- Undertakes clinical procedures
where dexterity and accuracy are required.
- Maintain a safe working environment
- Acts in such a way that safeguards
the health and wellbeing of children and vulnerable adults at all times. Is
familiar with and adheres to, the LTHT safeguarding policies
- Undertakes duties on other wards or
departments as and when required by service demands or patient need.
Responsibilities
for patient and client care
- Assesses, plans, implements and
evaluates care and treatments using specialist clinical knowledge and skills.
- Organises own clinical workload.
- Receives supervision of clinical
care and outcomes from Matron

- Refer patients as appropriate to
other members of the multi-disciplinary team
- Make clinical assessments within
clearly defined policies and protocols
Communication
- Maintains accurate clinical and
other records using the Leeds Teaching Hospitals Trusts documentation.
- Support patients and their families
communicating sensitive condition related information and ensuring they
receive required information to enable them to participate in their care
delivery
- Liaise with Community Services (as
appropriate) to ensure seamless provision of care delivery
Education and Training
- Provide training and education of
learners and other junior staff
- Participates in own and others
appraisals
- Provide education and training to
patients and their carers.
- Keep up to date with developments,
advances within the specialty
- Undertake mandatory and priority
training within the required time scales
Leadership and management
- Investigate and respond to
accidents, complaints, untoward incidents and other significant events
- Raise concerns with regards to
risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing
policy and supporting processes.
- Under the direction of Matron
participates in or leads the development of specific elements of nursing
practice, reflecting the CSU and Nursing business plans
- Actively promote and use the Leeds
Improvement method to improve the quality and safety of the service

- Promote evidence based practice,
wherever possible
- Engages users and carers views on
service delivery and development
- Implement policy and practice
guidelines specific to the clinical specialty, proposing changes where
appropriate
- Contribute to policy and protocol
development for speciality area
- Exercises a personal duty of care
in relation to equipment, resource and stock control utilising equipment and
supplies appropriately.
Research and Audit
- Contribute to the research agenda
- Undertake audit programmes specific
to client group, treatments and developments.
- Participate in clinical trials as
appropriate.

7. INFECTION CONTROL
The
jobholder must comply at all times with the Leeds Teaching Hospitals NHS
Trust Infection Control policies, in particular by practising Universal
Infection Control Precautions. Hand hygiene must be performed before and
after contact with patients and their environment.
8. HEALTH AND SAFETY / RISK MANAGEMENT
All
staff are responsible for working with their colleagues to maintain and
improve the quality of services provided to our patients and other service
users. This includes complying at all
times with the Leeds Teaching Hospitals NHS Trust Policies, including Health
and Safety policies, in particular by following agreed security and safer
working procedures, and reporting incidents using the Trust Incident
Reporting system

jobholder must take responsibility in agreement with his/her line manager for

jobholder will undertake all mandatory training required for the role.

12. SPECIAL WORKING CONDITIONS
i) PHYSICAL EFFORT:
There
is a frequent requirement for job holder to spend a substantial proportion of
their working time sitting or standing in a restricted position and an
occasional requirement to exert moderate physical effort.
ii)
MENTAL EFFORT:
There
is a frequent requirement for the job holder to concentrate where their work
pattern may be unpredictable.
iii)
EMOTIONAL EFFORT:
The
nature of the clinical and none clinical duties will require the job holder
to be involved in distressing or emotional circumstances on a frequent basis.
iv)
WORKING CONDITIONS:
The
nature of the clinical and none clinical duties will require the job holder
to be involved in occasional exposure to unpleasant or highly unpleasant
working conditions.
