Job summary

We are pleased to offer a permanent Band 6 Discharge Facilitator post working within Abdominal Medicine and Surgery (AMS). AMS is a large unit within St James Hospital comprising a dedicated surgical assessment unit and 12 in-patient wards. We are a regional centre for urology, colorectal, gastroenterology, upper GI, HPB and renal services, as well as being responsible for the Leeds Liver Unit, one of a handful across the country.

As a regional specialist centre our patients can often be extremely complex with a multitude of post-operative care and rehab demands. And due to our services being in high demand it is the responsibility of the discharge facilitators to ensure that once treatment has been completed, patients can be safely and efficiently discharged or repatriated back to the referring hospitals.

Organisational skills and an ability to effectively manage a caseload independently are a must. As are excellent communication skills and an ability to interact with not only all members of the multi-disciplinary team, but also patients and family members at often difficult and distressing times in their lives.

JOB PURPOSE/SUMMARY

The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist assessment care and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.

Current services comprise providing specialist advice support and training to ensure high quality discharge planning is undertaken by wards staff.

4. PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Fulfils the requirements as directed by the Nursing and Midwifery Council or Health Care Professions Council to maintain and revalidate registration

Clinical

Clinical care delivery

- Provide a clinical lead and role model to clinical staff by delivering high quality care.

- Undertakes clinical procedures where dexterity and accuracy are required.

- Maintain a safe working environment

- Acts in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies

- Undertakes duties on other wards or departments as and when required by service demands or patient need.

Responsibilities for patient and client care

- Assesses, plans, implements and evaluates care and treatments using specialist clinical knowledge and skills.

- Organises own clinical workload.

- Receives supervision of clinical care and outcomes from Matron

Freedom to act

- Refer patients as appropriate to other members of the multi-disciplinary team

- Make clinical assessments within clearly defined policies and protocols

Communication

- Maintains accurate clinical and other records using the Leeds Teaching Hospitals Trusts documentation.

- Support patients and their families communicating sensitive condition related information and ensuring they receive required information to enable them to participate in their care delivery

- Liaise with Community Services (as appropriate) to ensure seamless provision of care delivery

Education and Training

- Provide training and education of learners and other junior staff

- Participates in own and others appraisals

- Provide education and training to patients and their carers.

- Keep up to date with developments, advances within the specialty

- Undertake mandatory and priority training within the required time scales

Leadership and management

- Investigate and respond to accidents, complaints, untoward incidents and other significant events

- Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes.

- Under the direction of Matron participates in or leads the development of specific elements of nursing practice, reflecting the CSU and Nursing business plans

- Actively promote and use the Leeds Improvement method to improve the quality and safety of the service

- Implement policy and practice guidelines specific to the clinical specialty, proposing changes where appropriate
