Job summary

The Jobholder will carry continuing responsibility for a defined caseload, including the setting of standards of care, the assessment of care needs and the development, implementation, and evaluation of programmes of care. The jobholder will provide advanced clinical expertise to a specific patient group and will contribute to the development of the speciality service as well as contributing to the strategic direction of care within the Trust. The Jobholder will ensure cost effective use of resources.

Main duties of the job

The Sleep & Complex Home Ventilation Specialist is an experienced clinician who has specialist knowledge of respiratory conditions requiring on-going ventilatory assistance. The specialist clinician will assist in the discharge of patients who require this level of support, from any local Hospital to their own home, and provide on-going support to community teams and families so that they may continue to care for individuals at home. This includes the management of complex tracheostomy patients including training & completing tracheostomy changes and assessment, provision & training for patients requiring cough assist devices. The Jobholder will provide inpatient advice and support to Professionals and family when discussing home ventilation as a treatment option as well as advice and monitoring regarding stabilisation and titration of ventilation, and selection of appropriate equipment for home care. The post holder will work with multidisciplinary teams across Health, Education, Social Services, and the Voluntary Sector to facilitate discharge planning and provide on-going support for families in their own home and local community.

Job Summary

This is an exciting opportunity to work in the interesting and rewarding field of sleep disordered breathing (SDB). The Leeds sleep and complex ventilation service provides a comprehensive service to the people of West Yorkshire and beyond. We are a large centre with nearly 9000 CPAP users and over 1200 individuals on ventilation. The team comprises of a range of professions including Nursing, Physiotherapy and Physiologists all experts in their field and it is your turn to join them.

Based at St James's University Hospital in the vibrant and growing city of Leeds.

What you will bring to the role

The successful candidate will have some existing skills and will further develop skills in diagnosing & treating SDB, becoming an expert in CPAP, non invasive and invasive home ventilation.

Work is mainly outpatient based with a mixture of telephone and face to face clinics,

they will also provide specialist input across the inpatient base. Holding a clean valid driving licence and having access to a vehicle they will be expected to cover a geographical patch to provide their expertise at home and in other care settings as well.
