Job summary
The Community Children's Nursing Team (CCNT) in Gateshead are looking for a dynamic, hardworking Band 5 paediatric nurse to work with the team on secondment for a period of 12 months. The post holder will contribute to the delivery of an effective Children's Nursing service that is provided by a team of highly skilled practitioners. The team deliver clinical support to families, children and young people in their own homes or community.
Main duties of the job
Works collaboratively with other local health care professionals/providers to maintain high quality care and promote practice that is child and family focused within the locality.
To supervise students and delegate work appropriately according to skill mix ensuring maintenance of safety within the community.
To assess the health and health related needs of patients, clients, their families and other carers and identify and initiate appropriate steps for effective care for the individuals and groups.
To assess and treat specific diseases in accordance with agreed medical protocols.
To assess, plan, provide and evaluate specialist clinical nursing care to meet care needs of individual patients and carers in their own homes.
To oversee the provision of individual patient care by personal participation in order to maintain skills as an expert practitioner and provide an effective role model for the nursing team and others in the multi-disciplinary team.
To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
To work with Trust policies and procedures that ensure patient safety at all times
To work as specialist practitioner within a multi-disciplinary of an integrated workforce that is based in the community.
