Job summary
We are looking for a highly organised and compassionate Facilitator to join our dynamic Women's Health Gateway team at Gateshead Health NHS Foundation Trust. This is an exciting opportunity to play a key role in improving patient experience and supporting the delivery of high-quality, efficient care across our women's health services.
As Gateway Facilitator, you will ensure that patients receive a smooth, well-managed journey from referral through the Gateway Project. Working closely with clinicians, nurses, and administrative colleagues, you'll coordinate patient appointments, maintain accurate records, and act as the key point of contact for patients and families, offering support and information throughout their care pathway.
Main duties of the job
Key Responsibilitieso Coordinate patient pathways to ensure timely access to diagnostics, treatment, and follow-up care.o Liaise with multidisciplinary teams to manage referrals and clinic scheduling.o Provide compassionate and professional communication with patients, relatives, and healthcare staff.o Maintain accurate patient records, ensuring data quality and compliance with NHS standards.o Support service improvement initiatives within the Women's Health Gateway.o Coordinate communication of patients records with partner and external agencies.
