An exciting opportunity has arisen at Gateshead Health NHS Trust for a highly motivated individual to join the Tobacco Dependency Treatment Service 'QUIT Team'. The QUIT Team are responsible for ensuring that the NHS Long Term Plan commitments are achieved to reduce health inequalities and to tackle avoidable illness caused by tobacco use. The QUIT team recognise that tobacco dependency is an addiction and that it is a chronic, relapsing clinical condition that often starts in childhood and prematurely kills two out of every three long term users. Smoking is seen as a medical condition that can be treated, rather than as a lifestyle choice. The primary role of the post holder is to support abstinence from tobacco and to promote a long-term quit attempt for every smoker admitted to the smoke free, Gateshead Health NHS Trust and provide on-going support following discharge. The service reach includes Acute inpatients, the Maternity service, Staff and outpatients.
Main duties of the job
It will be commonplace for the post holder to support an inpatient during their hospital stay, visiting them on a daily basis on their ward with a view to provide Nicotine Replacement Therapy and offer evidenced based support to help with a personalised quit smoking treatment plan. The post holder will be expected to work collaboratively with the wider team to ensure that all smokers are offered support at the earliest possible opportunity and this will include pregnant people and their families and all inpatients within Gateshead Health NHS Trust. The post holder will be expected to use IT systems to document interactions in patients' electronic medical records. Home / community service clinics may be visited therefore it would be of benefit for the post holder to have a drivers licence. The post holder will be expected to contribute to team meetings and work proactively as a team member and also autonomously and independently. This role will include some weekend work to ensure appropriate service provision.

- Promote the Trust Smoke free Policy and support all patients, visitors and staff to be smoke free
- Have a keen interest in the welfare of others and work with people to change their behavior to improve their health in relation to tobacco use
- Act as a smoke free champion promoting the smoke free message through their day to day work
- Provide advice and support to ward staff and maternity staff to support inpatients to be smoke free and to support smoke free pregnancies following the NHS NENC ICB recommended model for smoking cessation
- Ensure robust discharge planning for the patients ongoing tobacco dependency treatment following discharge and provide on-going support to those patients for up to 12 weeks and beyond as necessary
- Document all encounters with patients using electronic and hand-written means to allow for continuity of care using transparent and contemporaneous record keeping which will evidence best practice
- To provide support for behavior change interventions in relation to tobacco smoking in line with NCSCT training and both regional and national pathways
- To support the day to day running of the Tobacco Dependency Treatment Service to ensure it is safe, well organised and evidenced based in line with NHS Long Term Plan
- Provide stop smoking support in a range of settings as required for the delivery of the Tobacco Dependency Treatment Service, e.g hospital, patients home, community services
- To support the delivery of the service key performance indicators and data reporting metrics for the NHS / ICS data reporting metrics
- Organise own workload and work both supervised and unsupervised in a busy team
