Job summary
An exciting opportunity has arisen to work as a Band 6 within team the Discharge Liaison Team in Gateshead Health NHS Foundation Trust. As a Discharge Liaison Nurse you will be expected facilitate safe and timely discharges of patients requiring a pathway 1-3 discharge from the Queen Elizabeth Hospital in Gateshead. The post holder will be required to deal with complex discharge cases and have an in depth knowledge of the various issues influencing hospital discharge. The post holder should have detailed knowledge of the NHS England delivery plan to recover NHS Emergency and Urgent Care services, specifically improving hospital discharge and improving joint discharge processes. The post holder should ideally have experience of working closely with social care services.
Main duties of the job
The post holder will be required to uphold the trusts ICORE values, through a period of change. The successful candidate will be able to demonstrate innovation, care, openness, respect and the ability to engage others.
The Discharge Liaison Team provides a hospital discharge service 7 days per week 8am until 8pm and the postholder must be able to work flexibly to meet the needs of the service.
Day to day duties include the following (please note this list is not exhaustive):
Co-ordinate and manage nursing/administration rota to provide a safe and effective hospital discharge service for pathway 1-3 discharges
Support hospital wards and discharge co-ordinators
Attend and facilitate training
Support students/visitors to the hub
Community visits to support discharge where applicable
Attend MDT meetings to facilitate the safe discharge of complex cases
Support the hospital with expert advice on housing/homeless concerns
Liaising with safeguarding where appropriate
Onward referrals
Supporting hospital therapy teams to facilitate discharges
Work alongside therapy teams to arrange environmental visits and equipment needed
Liaise with ICB
Work closely and in partnership with Gateshead Local Authority
Complete trusted assessments for pathway 1 referrals
Support end of life pathway 3 discharges
