Job summary
Gateshead Health NHS Foundation Trust is pleased to be advertising the post of a system pathway navigator to support the regional tertiary gynae-oncology service based at Gateshead Health NHS Foundation Trust. This is a full time post, and the post is based on site at the Queen Elizabeth Hospital.
The successful applicant will work closely colleagues from across the region in both tertiary, secondary and primary care. The successful applicant will support patients throughout their treatment journey, as they move between specific services provided by different Trusts. As a system pathway navigator, the successful candidate will be expected to work across organisational boundaries to ensure patient's pathways are seamless, and patients are kept informed throughout their journey. The post holder will work closely with Cancer Nurse Specialists from GHNFT and different organisations.
Main duties of the job
Work as a core member of the speciality admin team alongside the clinical team to support all aspects of the patient pathway from referral to treatment.
Work with the MDT coordinator and clinical team and cancer teams to support the specialist MDT, ensuring appropriate patient documentation is available and actions from MDT are progressed in a timely and effective manner.
Identify and escalate potential breaches to lead clinician, service line manager/assistant service line manager and relevant admin/cancer tracking team members with details of the breach and actions taken to resolve.
Work closely with SLM/ASLM, service line admin team, cancer tracking team and clinical team to support the Cancer Harm Review process.
Work closely with cancer services, Information and Performance teams to ensure accurate and timely data is available.
Support Trust and network wide pathway developments for cancer services. Support regional and national reform initiatives to improve the quality and speed of patient access to consultation, diagnosis and treatment.
As the role is embedded within the Trust, an element of staff management will be included in this position.
