Job summary
We are advertising three Consultant Geriatrician posts to join our department. All will involve care for patients on our Care of the Elderly wards.
This post are available to enhance and support the Elderly Medicine wards within the Jubilee Wing and to support a developing acute geriatrician service as part of the Trust's Same Day Emergency Care strategy. The posts will be for 10 programmed activities (PAs) per week with an additional 0.71 PAs available for programmed specialty weekend work. The appointee would support the current elderly medicine service and work alongside established Consultant Geriatricians whose interests include falls and syncope, Parkinson's disease, osteoporosis, rehabilitation & community geriatrics, mental health in the elderly and acute frailty.
Main duties of the job
The Care of the Elderly department operates an Acute Frailty service based in the hospital's Emergency Admissions Unit (EAU). A dedicated multi-disciplinary team of Consultant Geriatrician, Specialist Frailty Practitioner Nurse, Frailty Fellow Doctor, Physiotherapist, Occupational Therapist, Therapy Assistant and Pharmacist, all work together to assess and manage older, frail patients admitted under medicine. Daily morning ward rounds by the Consultant Geriatrician are complimented by the multi-disciplinary teams review of additional patients throughout the day. The service maintains close links with community teams, the COTE virtual ward and the COTE back of house wards to optimise patient experience and reduce length of stay.
We also run a COTE Virtual Ward. Here we provide Consultant Geriatrician led care to patients in their own homes with acute medical needs. Patients can be referred to the virtual ward from the community and secondary care. The Virtual ward team includes Specialist Frailty Nurses, an Occupational Therapist and Physiotherapist, all of whom can visit patients in their own homes. The Consultant Geriatrician leads the care of the patients through two daily MDT meetings, reviews of potential admissions to the ward in the community and in hospital and home visits to assess and manage patients in the ward.

In addition, there are opportunities for working closely with community teams, the COTE virtual ward and the wider medical staff based on EAU.
A further post gives the opportunity to spend multiple days leading the Care of Elderly Virtual Ward. This includes assessment and management of patients both in the hospital and with visits out into the community. Work will be alongside a dedicated MDT in a service we hope to expand and develop to increase patient capacity over the coming years. The successful candidate will be expected to take a role in service development and evaluation, as well clinical support.
Our third post provides the opportunity for a Consultant Geriatrician with a specialist interest in Falls and Syncope and/or Osteoporosis to expand our outpatient services in these areas. Our Falls and Syncope service is led by multiple Consultant Geriatrician outpatient clinics. These clinics are supported by dedicated Falls Specialist Nurses and Tilt-Table equipment on site. In addition, the Falls Specialist Nurses provide assessment of patients in the community and feedback into a weekly MDT. The successful candidate will have the opportunity to expand the current service through their own specialist area of interest. Our Osteoporosis service is also a multi-disciplinary team service led by Consultant Geriatrician and Specialist Nurses. DEXA scanning services are available on site. The service operates multiple weekly outpatient clinics and takes bone health referrals from around the region. A successful candidate would be able to expand the existing service and lead in its future development.
