Job summary

We are seeking to recruit a Band 5 Community Nurse to join our Younger Persons Memory Service at Bensham Hospital. Applications from Mental Health Students who have an interest in this area and are about to qualify will be welcomed.

This service offers pre and post diagnostic support for people aged 65 and under who have been diagnosed or are having investigations for dementia. We also offer evidence based therapeutic intervention for patients who are diagnosed. This is an exciting time to join the service as we are currently undertaking service improvement to provide a high standard of responsive care to patient and families.

Main duties of the job

You will work autonomously, carrying your own caseload of patients who have been diagnosed with a young onset dementia, providing evidence based treatment and post diagnostic support. You will work within the community as well as on site and deliver some group therapy sessions; this will require you to work with community agencies. Supervise student nurses and work with all members of the multi disciplinary team. Working collaboratively with external agencies. Complete outcome measures. Participate in audit.
