- An exciting opportunity has arisen for a Band 7 Nurse who is passionate, highly motivated, and who demonstrates the Vision of our Trust, to become part of our Diabetes team.
- To work as an integral member of the Diabetes team.
- Support the provision of specialist Diabetes nursing knowledge, advice and education to other nursing staff, the wider multi-professional team, allied health professionals, and patients .
- Support the provision of education and training for nursing, allied health professionals and other health care professionals to promote insight into the specific needs of adults and young people with diabetes; including health promotion.
- The post holder will be part of a team who provides high quality Diabetes nursing care.
- Required to work flexibly in order to support the needs of the service.
Main duties of the job
- To clinically lead, develop and coordinate the delivery of the integrated diabetes service through the use of effective evidence based practice in line with agreed national standards for patient safety and clinical practice/excellence.
- To provide the highest standard of holistic care for patients through the utilisation of advanced clinical skills and advice in the assessment and management of patients, working autonomously and with the multi-disciplinary team to improve the quality and safety of patientcare.
- To work autonomously and be accountable for the planning and delivery of all aspects of patient care in relation to current national best practice guidance, and as set out in the NMC code.
- To continuously improve clinical care standards in line with the clinical governance framework.
- To ensure the highest quality of care is provided in the safest mannerTo provide clinical leadership, expertise and specialist clinical advice and support to clinical staff on the management and care of patients with Diabetes and lead on the effective delivery of care to patients.
- To lead the in provision of expert specialist knowledge, care and experience to support to the wider nursing team to facilitate the effective delivery of care to patients at all times.
