An exciting opportunity has arisen for a Band 6 Nurse to join the Community Stroke Rehabilitation team at Bensham Hospital.

This is an opportunity to develop your understanding of the stroke pathway and your skills working with patients in a community setting. You will work as part of a multi-disciplinary team in a community setting to support patients following a stroke.

The successful candidate will have the opportunity to further develop quality expertise and lead projects and pieces of work by developing strong relationships and network across the region. You will work collaboratively with the wider multi-disciplinary team, other health and social care practitioners and the nursing staff, in order to provide the best care and quality outcomes for patients and service users, in-keeping with national guidelines.

Main duties of the job

This post is a Band 6 nurse with the community stroke rehabilitation multi-professional team within Gateshead.

The post holder will provide specialist nursing with a defined caseload of stroke rehab patients. You will provide comprehensive holistic nursing assessments for community patients including obtaining patients observations, assessment of care needs, secondary prevention, self management strategies, mood and assisting in the implementation of rehab programmes.

Provide ongoing input when required for interventions such as monitoring blood pressure and support with mood. Liaise and refer on to other professionals and organisations such as GP's, health navigators, social services, psychology, stroke association, Age UK etc.

You will be involved with triaging patients and identifying the appropriate initial team member to visit the patient and carrying out 6 month reviews.

This post is designed to support the Community Stroke Rehab team and the Team Lead fulfil their objectives, ensuring the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting.
