Job summary
This post is for a Specialist Occupational Therapist or a Specialist Physiotherapist, working within the Hospice at Home and Specialist Palliative Care multi-professional team within Gateshead. To ensure the effective delivery of therapy interventions into all patient areas working flexibly over 7 days per week.
This post holder will also be the Team Lead responsible for the Hospice at Home team offering a compassionate leadership approach to supporting the 3 band 6 Nurse Coordinators and 27 band 3 Health care assistants. Ensuring effective strategies are in place to offer quality care and data collection of service delivery.
Main duties of the job
To work effectively within a multi-disciplinary/agency team managing patients identified as requiring an urgent or rapid intervention to prevent their health and wellbeing deterioration. In addition, there will be planned activity to provide hands on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care and to maximise patient independence.
This post is designed as a senior clinical autonomous advanced therapist who will ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty.
The autonomous practitioner will provide comprehensive therapy interventions and plans sensitive to the needs of the population, responsible for senior clinical decisions, including advanced therapy assessments, taking history, diagnosing, planning treatment, and evaluation of care. Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in Gateshead and will be responsible for coordinating and meeting the health care needs of people at risk of hospitalisation or requiring a more intensive level of care in the unscheduled, intermediate, palliative and EOL care environment.

- To participate in the planning, development and evaluation of clinical services using advanced clinical skills and knowledge.

- The post holder will work within the Hospice at Home team and the Specialist palliative care Team, they will also liaise with multiple stakeholders in order to ensure that the day to day co-ordination and delivery of service is in a timely and professional manner.
- The post holder will play a key role in supporting the Hospice at Home and Specialist palliative Care Team with a range of therapy interventions which enable the patient to receive the optimum appropriate health care outcomes. There will be a need to understand and undertake administration and co-ordination of tasks within the Hospice at Home Team and to delegate to other members of the team as appropriate.
- The post holder will work with independently demonstrating the ability to use initiative, utilise IT systems and ensure effective and timely in a professional manner.
- To provide clinical leadership for junior staff and students through Supervision, support, education and appraisal.
- Working in collaboration with all members of the MDT and external agencies to develop the Hospice at Home Service. This will include attendance at MDT meetings, ward rounds, case conferences and discharge meetings.
- Take a lead role in implementing and undertaking audit and practice development of staff at all levels.
- Work as an autonomous practitioner, which will require making assessments, the formulation of therapeutic plans and monitoring for patients including those with more complex needs.
- Maintain timely and accurate patient records in line with Trust Policy as an Autonomous practitioner.
