Job summary

The post will be part of the integrated Specialist Palliative Care Nursing team working closely with the Specialist palliative care nurses and the Hospice at Home team. The specialist nurses work closely with acute wards and departments, GP practices, Care homes and the multidisciplinary teams. The post holder will work collaboratively with Macmillan.

This post will be a valued addition to the Education team working alongside the Lead end of life facilitator and further part time end of life facilitator.

As a Band 7 End of Life Care Facilitator there will be managerial and leadership responsibilities particularly to Band 6, junior staff and students.

There is an expectation that the post holder will support the Lead End Of Life facilitator in the strategic planning of education and training to a variety of stakeholders.

Main duties of the job

  • To be responsible for ensuring that agreed professionals standards are reflected in clinical practice.
  • Be involved in changes in clinical practice, delivering the strategic vision.
  • Support and coordinate the delivery of education and development of End of Life Care through the use of effective evidence based practice in line with agreed national standards for patient safety and clinical practice.
  • Provide clinical leadership to nurses and other healthcare professionals across the Trust.
  • To embed innovative patient centred service within education utilising advanced skills and knowledge in palliative and end of life care.
  • To apply standard principles of infection control to all activities at all times.
  • Be a source of expert knowledge and expertise in palliative and end of life care to multiple stakeholders including patients, their families and carers.
  • To take the part in the co-ordination and delivery of the agreed education program for palliative and end of life care across community, acute and care home settings.
  • Work clinically in line with service demands offering specialist palliative care nurse assessment to patient in community and acute setting. Offer advice and guidance to wider professional groups.
