An exciting new opportunity has arisen to work within the lung cancer team based at the Queen Elizabeth hospital in Gateshead. We welcome applications from highly motivated, caring individuals who have a passion for working in cancer care.

The team consists of 5 chest physicians, 4 lung cancer nurse specialist, 1 lung cancer support nurse and 2 oncologists. The team is supported by 2 consultant radiologists and pathology services. The team work closely with primary health care teams, specialist palliative care teams both in primary and secondary care as well as with allied health care professionals ensuring the best possible outcomes for lung cancer patients in Gateshead. The role of the Macmillan lung cancer care co-coordinator will enhance the work already achieved and will further improve the patient experience. The post holder will work primary with the lung cancer nurse specialists, providing a proactive, timely, supportive service to people at all points in their lung cancer journey.

Main duties of the job

  • Follow up patient enquiries on a daily basis liaising with the lung cancer nurses, initiating appropriate response according to protocols and individual pathways.
  • Use good communication skills, basic clinical awareness, utilising appropriate tools and procedures, liaising with the lung cancer nurses as appropriate.
  • Refers complex decisions regarding patients to the specialist nursing team for assessment and review.
  • Daily use of systems to document and monitor all aspect of patient care coordination.
  • Supports data collection for service delivery, audit and quality surveillance.
  • In conjunction with lung cancer nurses will discuss and coordinate the care for lung cancer patients.
  • Organise and prioritise a designated workload in relation to identified needs.
  • Support the lung cancer nurses in collection of appropriate information to complete the holistic needs assessment documentation for a range of cancer patients.
  • Communicate effectively with appropriate teams ensuring safe and effective transition of patient care between services and relevant information shared.
  • Support the lung cancer nurses in delivering patient-centered, self-management support and education as necessary to non-complex patients, including how to self-assess (an understanding of what signs, symptoms or situations to be aware of that would indicate concern).
  • Promote telephone advice line for patients/carers to use when they feel that their condition or needs have changed.

Job responsibilities

  • Direct patient and carer contact.

  • Assists with tracking of relevant diagnostics test results and flags to lung cancer nurses.

  • Take responsibility for the maintenance of accurate paper and electronic information
  • Resources using systems where appropriate and ensuring confidentiality.
  • Work as an effective team member, providing support for other members as required to maintain service provision.
  • Leads preparation for cancer of lung cancer MDT ensuring all information correlated and presented for meeting in timely response.
  • Supports admin of all meetings undertaken by lung cancer nurses.
  • Inputs required data relevant to service delivery via appropriate systems e.g. Dendrite.
  • Participate in data audit activity/surveys aimed at the provision of quality cancer care.
  • Adheres to national recommendations ensuring communications and governance are maintained at all times promoting patient safety, high quality specialist clinical care and advice.
