Job summary
We are looking for an enthusiastic and motivated General Adult Consultant Psychiatrist to join our Community Rehabilitation Team. The Community Rehabilitation Team is a well-established multidisciplinary team delivering a community rehabilitation service across south west London to patients with severe and enduring mental illness in a variety of settings including out of area inpatient rehabilitation units, high support residential placements, acute inpatient units and community teams.
The community rehabilitation team forms part of the complex care pathway in South London and works closely with the trusts two rehabilitation inpatient units, Phoenix Unit (High Dependency) and Burntwood Villas (Community Rehabilitation) but also supports the South London Partnership (SLP) Complex Care Programme in a number of areas including:
- Managing step down from out of area inpatient rehabilitation units
- Ensuring high quality care is evident in referrals to the SLP Single Point of Access (SPA) and working closely with the SLP Clinical Assessment Team
- Supporting existing rehabilitation providers such as the One Housing Group and Bridge24.
- Supporting the flow of care through the system
- Maximizing community stakeholders including the voluntary sector to enable full recovery and independence for our service users
Main duties of the job
The post-holder will provide medical leadership to the community rehabilitation team and will work closely with the Team Manager to ensure a safe, efficient and effective service that meets the Trust's Key Performance Indicators. The team consultant will carry overall responsibility for the day-to-day clinical care, risk assessment, risk management, initiation of medication, and the recovery focused care delivered by the Team. The Consultant will be afforded robust support by the Clinical Lead for Complex Care, the Clinical Director for Community Services and where necessary the Medical Director and Chief Executive.

- Ensuring the delivery of effective, efficient and safe, recovery focused mental health care.
- Medical leadership, working in the multi-disciplinary team and working closely with the Consultant Psychiatrists and other teams (around referral and liaison) and occasionally their Associate Clinical Director within their borough or service line.
- Delivery of the day-to-day clinical care, risk assessment, risk management, initiation of medication, and the recovery focused care delivered by the Teams
- Attending weekly clinical team meetings and performing medical duties that are decisions of that meeting
- Availability within normal working hours 5 days a week for urgent review and management including formal Mental Health Act assessments.
- Multi-disciplinary and multi-agency working with social care, statutory and voluntary and third sector partners.
- Compiling a patients history from a number of sources, and preparing case summaries and discharge summaries
- Compile reports for MHRT and attend on behalf of RC when this deemed appropriate (CTO patients).
- To take part in the discharge planning, CPAs and advising primary care colleagues.
- Liaising with other clinical teams within and outside the trust
- Liaising with general practitioners and medical specialists with regards the physical health of the patients
- Undertake urgent psychiatric assessments when necessary.
- Domiciliary visits as required.
- Ensure effective and timely documentation and communication of clinical decision making and management plans

Qualifications
Essential
- Primary Medical degree.
- Full GMC registration with licence to practice.
- MRCPsych, or equivalent.
- Included on the GMC Specialist Register for General Adult Psychiatry or eligible for such inclusion on the Specialist Registrar within three months of completing their CCT.
- Section 12 (2) approval of the Mental Health Act
Desirable
- Qualifications in another other specialism.
- Qualification in a talking therapy such as CBT, DIT or MBT
- Qualifications in change management and service development skills
Knowledge: Critical Expertise in Specialty

- Completed minimum of three years full time training in approved higher psychiatric training scheme.
- A detailed knowledge of main treatment approaches for severe mental illness including psychopharmacological interventions.
