Job summary
An exciting opportunity has arisen to be part of a new pilot project. As part of the reinvestment of savings made by the SLP Complex Care Programme, a new community rehabilitation project provided jointly with Bridge Support is opening in the Borough of Kingston in early 2023.
The Community Rehabilitation Pilot will provide specialist enhanced rehabilitation support to adults typically aged 18-65 diagnosed with severe and enduring mental health needs who would benefit from an intensive rehabilitation and recovery-focused approach which will best be met by an integrated clinical and voluntary sector shared model of care and support within an accommodation-based service.
The accommodation and support staff are provided by Bridge Support in the form of two six bedded properties in the Royal Borough of Kingston. SWLSTG will be providing dedicated medical, occupational therapy, psychology and co-occurring mental health, alcohol and drugs practitioner input and the posts will sit within the already established Community Rehab Team, based at Trinity, Springfield Hospital. Although based in the borough of Kingston the pilot will be open to service users from all five boroughs served by SWLSTG
Main duties of the job
The candidate will need:
- The successful candidate will therefore have experience of working within a rehabilitation setting, have a strong commitment to improving the recovery outcomes of individuals, and enjoy working with stakeholders to support service transformation and innovation.
- To manage a caseload of clients with complex needs, using evidence based /client centred principles to assess, plan, implement and evaluate interventions.
- To offer OT intervention for patients living at the SWLSTG Rehab Community Pilot Provision team caseload, being responsible for completing standardised assessments / manualised interventions (including groups) as agreed by senior professional lead
- To provide leadership for junior staff, through supervision and appraisal.
- To participate in the planning, development and evaluation of OT services within the Bridge Rehabilitation Team, holding responsibility for defined projects
- To contribute to the maintenance and development of the Trust OT Service
- To regularly supervise OT students on practice placement
- To contribute to streamline working from admission to discharge in keeping with the principles in the Rehabilitation / Recovery Care pathway.
- To promote recovery and social inclusion.
- To act as the designated 'Disability Champion' leading the multi-disciplinary team in Disability Equality matters within the service and other areas as required.
