Job summary

An exciting opportunity has risen for a passionate and enthusiastic Band 5 Mental Health Nurse on Jasmine Ward (Tolworth Hospital, Surbiton).

Jasmine Ward is a 16 bedded unit that offers care and treatment for male and female patients over 65 years of age. The service offers Inpatient care for older people with mental health problems, provides assessment and treatment of older people with a range of diagnoses, who require hospital care due to the severity of need. We provide services for patients who cannot be cared for in the community or other settings, due to the intensity and expertise of the care required. The ward is located in Surbiton, with good local links via train and bus to both inner London and wider Surrey areas.

Within this setting all aspects of physical health, social care needs and risks are jointly managed by an excellent multi-disciplinary team, which forms part of a planned and integrated holistic system approach to care.

Main duties of the job

The post holder will deliver high quality nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.

The role includes leading the delivery of comprehensive service user assessment, planning, implementation and evaluation of person-centered care in a collaborative way. The post holder will co-ordinate a first class service that includes assessment and monitoring of physical health, maintaining service users rights, supervising junior staff and students, working collaboratively with the Multi-Disciplinary Team and maintaining a safe and therapeutic working environment and accurate records.

You need to have a genuine passion in working with elderly patients and willing to make a difference in their lives. This role is pivotal in supporting the team in delivering care. You will be expected to deliver high standards of care incorporating the recovery approach to cover all aspects of health needs and risks within a safe environment by working closely with the relevant community services.
