Care Co-ordinator (CPN/OT/Social Worker) - Twickenham Recovery and Support Team (Band 6)
A great opportunity exists with a vacancy arising for the role of Care Coordinator in the Twickenham Recovery and Support Team. We are open to applications from Occupational Therapists, Community Psychiatric Nurses or Social Workers. The service offers support to adults aged 18-75 with a range of mental health conditions.
The successful applicant will have scope to develop as a Clinician, in a multi-disciplinary team. Care Coordinators reports the Team Manager who reports to the Clinical Manager for Adult Community Services in the Borough of Richmond.
The successful applicant will be clinically responsible for a caseload of up to 30 patients. They will participate in key meetings, seeking to provide high-quality services strongly focused on recovery to patients. The role requires one to attend to commissioned Key Performance Indicators (KPIs) for activity and quality.
Main duties of the job
We are looking for a positive, motivated and committed individual who understands the Trust's values and their role in creating a vibrant work culture. This post is open to those professionally registered with the HCPC, NMC or Social Work England. It is desirable for applicants to have experience of working cases of patients within a mental health community setting.
We are also open to those who wish to develop their careers and would be happy to consider a band 5 - 6 developmental role. You would be supported by the team to complete a set of competencies and with additional Trust-wide support with reflective sessions and working groups to develop your skills.

About us
The Twickenham Recovery and Support Team is multi-disciplinary team who works in partnership with multiple other agencies: Adult Social Care, Housing Associations, Probation, Voluntary Sector.
The Team works from the Maddison Centre close to the vibrant Teddington high street and the river Thames with a range of shops, cafs and restaurants. The hub is located a short walk from Fulwell and Teddington stations. They also see patients at Tolworth Hospital in borough of Kingston close to a number of shops and with good transport links close to the A3 and a bus ride away from Surbiton station, which has fast connections to Clapham Junction and London Waterloo.
