Community Mental Health Nurse (Band 6)

Kingston Older People's CMHT & Memory Service

South West London and St George's Mental Health NHS Trust has a rich history, well-established community links and an international reputation. We provide a full range of local mental health services to the Kingston, Merton, Richmond, Sutton and Wandsworth London boroughs, as well as specialist forensic mental health services, including high secure services, at Springfield Hospital.

We take pride in providing specialist care to our service users where our Trust values; being consistent, compassionate, collaborative, respectful and open is at the heart of everything we do. When you join us, you'll be part of something special - an innovative and rapidly growing organisation that is helping to redefine the Mental Health, education and social care landscape in the UK.

Main duties of the job

The role of Community Mental Health Nurse is a demanding yet rewarding one. It requires adopting a recovery model of care, autonomous case load management and helping to manage service user's risks and needs. As a older adults mental team we are exclusively looking for candidates with a mental health nurse qualification (NMC registration) and experience of working with our client group. We would also encourage candidates to apply who may have a strong desire to work with older adults where support and training can be provided. Average caseloads are around 30 service users and involve joint care planning with carers, relatives and other organisations who provide care. You will be supported by an experienced clinical team leader and three consultant psychiatrists, psychologists and dementia specialist nurses. This is a community post and the successful candidate will be expected to complete home visits, so car driving is essential. The post holder will also be expected to support the team's duty worker system - helping to provide cover for the service in the absence of other colleagues. The service opening times are 9am to 5pm, Monday to Friday. This is a full-time post for 37.5 hours per week. There may also be some opportunities to perform some duties while working from home.

About us

The Kingston Older Peoples CMHT and Memory Service has been well-established for over 20 years. In the past few years the service has been transformed from an integrated team to a health only service therefore outsourcing social care provision. During the new formation of the team many new roles have been developed. All our staff work across both the Memory Service and Older People's CMHT and we enjoy integrated caseload management, helping to better meet our service user's needs and manage risk.

Tolworth Hospital has good transport links to London via Surbition and Tolworth Railway Station. There is also several buses that service the local high-street and a bus from Kingston and New Malden that stops outside the hospital. Both Tolworth and Surbition have major high street chain shops and restaurants. Parking for staff is available at subsided rates. The Tolworth Site is also undergoing extensive rebuilding work to provide a better experience to our service users and working environment for staff.

Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK.
