Job summary

Full time Band 5/6 Development or Band 6 Opportunity within Sutton Older Peoples Community Mental Health Team

We are looking for an enthusiastic Mental Health Nurse or Occupational Therapist to join our Sutton OP CMHT, which sits within the wider Older People's Team, including Memory Assessment, Intensive Community Support Team and Care Home Liaison Team. We are based at Cheam Resource Centre in North Cheam with working hours of work Monday to Friday 9-5.

Main duties of the job

The post is available to those seeking a Band 6 position or wishing to develop into such a position within a supportive and extremely experienced team. Applicants need to be professionally qualified and committed to providing responsive and high-quality clinical services to mental health service users and their carers.

This role is for a Care Co-ordinator to work with patients with cognitive impairment and functional illnesses and so will need relevant experience of working with Older People, ideally within a within a range of settings. Applicants must have a keen interest in all mental health issues affecting Older People and therefore is required to have up to date knowledge of treatments and protocols. The post holder will be able to work autonomously, exercise clear and coherent decision-making skills and be able to work flexibly to meet the needs of the wider service. The post sits alongside experienced clinicians of various disciplines, both within the CMHT as well as other sub teams.

The role involves working closely alongside the patients and their carers as well as various relevant agencies, including GP's, acute Hospitals, Social Services, Admiral Nursing as well as voluntary support services.

As part of the team you will be expected to participate within the duty system.
