Job summary
The Coral Crisis assessment hub is co-located with The Lotus assessment suite at Springfield Hospital and covers five boroughs Wandsworth, Sutton, Merton, Richmond and Kingston. The crisis hub includes the following teams, the Mental health support line, Street triage practitioners and the crisis assessment team (CAT). The unit provides emergency same day mental health assessments for patients presenting in crisis. The unit includes The Lotus assessment suite consisting of 7 reclining chairs and two Coral plus chairs for patients who cannot be safely assessed in a community setting.
The service aims to provide a single point of access to patients in crisis. The hub will be able to conduct same day face to face emergency assessments in the community. The hub will work closely with partner agencies such as the police, NHS111 and LAS to ensure patients presenting in crisis receive timely, accessible, safe, effective, high quality care. The service will provide same day urgent assessments to patients over the ages of 18yrs old and telephone support to children and YP from all boroughs within the Trust.

About our location:
Tooting (Wandsworth)
Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK. This is easily accessible via the Northern Line (Tooting Bec Station) and via private transport. The Trust provides subsidised parking fares for all our substantive staff. Our main site is located immediately next to Tooting High Street where staff can easily access a range of shops, cafs and restaurants.

The post holder will be a senior practitioner within the Trusts Crisis assessment hub. The Crisis assessment hub is an integrated assessment service. It provides mental health assessments for patients in acute crisis in the community requiring same day emergency care. Patients can be referred via the Trusts Mental Health Support Line 24 hours a day, 7 days a week. The service aims to work in partnership with the police andambulance services. The Crisis Hub has the ability to assess patients in their own home, also to consider alternatives such as going to the Recovery Caf to be assessed there, or to local healthcare premises such as a GP surgery or walk-in centre to be assessed there. Or in one of the two Hub chairs.
Through the provision of an informed assessment of needs/risk, and the enabling of the right community support to meet these where feasible the Crisis Hub aims to:
- Transform the crisis pathway to ensure patients in crisis receive high quality care, in the least restrictive environment possible.
- Provide timely access to services that aim for parity of esteem with physical health
- Provide access to an all-age 24/7 mental health crisis line, that includes transfer from 111 and GP services.
- Provide face to face services that provide support in the most appropriate setting, which may include mobile services
- Provide access to services that are supported by effective direction to / transport between services
- To access provision outside of an emergency department setting, unless there are over-riding physical health needs.

- RMN (current NMC registration)
- Evidence of continuing professional and personal development
- Mentorship Qualification
- ENB 998 or equivalent
Desirable
- RGN
- oEducated to degree or diploma level nursing or other courses relevant to acute and urgent care e.g. brief solution focussed therapy, risk assessment/management, assessment etc.
Experience and Knowledge

- oDemonstrable experience of post qualifying work in psychiatry, with evidence of experience at Band 5
