About the team:
The Mitcham Recovery and Support Team is a community-based service for working-age adults between 18 to 70 years, who have experienced severe and enduring mental illness.
The Mitcham Recovery and Support Team works collaboratively with clients, their carers and networks with the aim of minimising the impact of mental disorder has on their lives.
The team is made of integrated health and social care professionals. We work in close partnership with GPs, Housing, Police and other key partners, including voluntary organisations.
Mitcham RST is a stable, cohesive, supportive team built on mutual trust and collaboration. Teamwork is important to the service, so we are looking for someone who shares the same values.
We currently have one vacancy because we have created a substantial team that allows individuals to grow and progress. The team is currently working flexible hours which you may find rewarding.
About the location:
The position is open within the Mitcham Recovery and Support Team which is located within the Wilson Hospital, Mitcham. The Wilson Hospital is easily accessible and has good transport network and link to Trains, Trams and buses, all within 10 minutes walk. There is free car parking on site and the Trust offers reduced rate of parking on other sites.
Main duties of the job
An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse (CPN) x 2 to join the Mitcham Recovery and Support Team. If you are passionate about impacting positively on the lives of people with severe and enduring mental illness in a community setting, then this could be the perfect job for you.
The successful candidate will be able to offer intensive engagement work and recovery-orientated interventions. You will hold a caseload and work with a complex caseload of clients with varied mental health conditions. The role primarily involves developing treatment and interventions for health conditions within a recovery model of care. Successful candidates will need to demonstrate an understanding of client's health and social care needs.
You will be able to develop and enhance the therapeutic care of individuals within the community. The role involves among other things, completing comprehensive assessments, risks assessments, drawing up meaningful care plans, risk management plans and delivering appropriate and therapeutic interventions. You should be committed to MDT and partnership working, establishing good networks with colleagues in the HTT, RST, In-Patient services, Police, GPs and a variety of voluntary, statutory and independent agencies.
