Job summary

We are seeking a motivated, Highly Specialist Physiotherapist to join our community Learning Disability Healthcare Team. As a friendly and dynamic team we provide specialist services for adults with learning disabilities across Wandsworth.

You will need specialist clinical assessment skills and the ability to work with people who present with complex physical disability. You will be involved in 24 hour postural management, MDT working in dysphagia management, mobility assessments and falls prevention, as well as optimising exercise opportunities for adults with learning disabilities. Assessment and intervention will take place in a variety of community settings including clients' own homes, supported living facilities, registered care homes and community day services.

You will not only carry a varied caseload but will also provide clinical leadership to a small team of physiotherapists and assistants. You will be supported in this aspect of the role by the Clinical Team Leader.

The successful applicant will work closely with the multidisciplinary team comprising of community nursing, dieticians, occupational therapists, speech and language therapists, psychologists and psychiatrists. The post holder will also develop excellent working relationships with other NHS services and external agencies.

Main duties of the job

  • To offer interventions that are person centred, respecting choice and values.
  • To use a problem solving approach, identifying and managing risk, and working in the most appropriate environment for service users and their carers ensuring equitable and accessible service provision.
  • To independently manage a caseload of service users with complex health and social care needs.
  • To provide professional and clinical leadership for the physiotherapy team
  • Ensure physiotherapy team meets agreed targets and objectives
