Community Psychiatric Nurse - Memory Assessment Nurse - Band 6

An exciting opportunity has arisen for a Community Psychiatric Nurse to work in the Richmond Memory Assessment Service. This is an opportunity to be part of a dynamic multi-disciplinary team that assesses all referrals for old age adults with mental health needs and resident in the borough of Richmond.

Main duties of the job

This post will entail working as part of a well-established MDT and the post holder will be required to offer holistic assessments and manage a caseload of patients. Part of the role will be liaising with other stakeholders such as GP's, Local Authority and other voluntary organisations to develop good relationships. The post holder will develop skills in assessment, planning, implementation evaluation and caseload management and also know to appropriately signpost referrals to other services or Recovery / specialist teams.

We are looking for an enthusiastic, positive, motivated and highly committed individual with good communication skills, who can work with us to support the work of this vibrant team. We encourage applicants who are keen to develop their skills in assessments and caseload management who enjoy working autonomously but with the support of the team. You will work with a Recovery Model and be fully involved in zoning and MDT discussions.

About us

The post is based five days per week in the Richmond Older Peoples' Community Mental Health services at Barnes hospital.

The post is based at Barnes Hospital. Barnes Hospital is easily accessible from Mortlake, Barnes and Barnes Bridge railway stations with frequent trains to London Waterloo, frequent local buses, and via private transport. There is no canteen on site but there are local independent shops and restaurants easily accessible within a ten minute walk. The site is also a fifteen minute walk from the River Thames.

The Trust provides subsidised parking fares for all our substantive staff and parking is available on site and in the local area.

The Richmond Older Peoples' Mental Health Service comprises the Recovery & Support Team, the Intensive Community Support Team, the Care Home Liaison Service, and the Memory Assessment Service. These teams provide assessment and treatment to clients age 75+ with severe and enduring mental health needs in the community, outreach support to nursing and residential homes and day centres in meeting the complex and challenging needs of people with dementia and other mental health needs, and also assessment and diagnosis of clients with suspected dementia of any age.

The Band 6 Community Psychiatric Nurse/Memory Nurse will be involved in screening and triaging incoming referrals to the Richmond OP Service to assess/determine what the individuals mental health needs are, identify potential risks, develop an initial care plan and develop a collaborative response to needs. They will be required to undertake urgent assessments to determine need.

  • Act autonomously to identify the most appropriate course of action, including urgent/emergency situations, ensuring effective transfers of care and escalating to the Team Leads/ Manager or Consultant as appropriate.
  • Act as a Care coordinator for a caseload of patients managing their Zoning, Care plans, risk assessment, collaborative and crisis plans
  • Demonstrate the skills to professionally communicate complex information to users and carers regarding bio-psychosocial problems in an empathetic and supportive way
  • Provide clinical and professional advice, leadership supervision and support to junior staff, patients and families/ carers.
  • To liaise with professionals/ agencies and ensure the patient receives continuity of care/ service through clear and timely communication with all involved in their care.
  • To undertake initial assessments where this is indicated to assist in information gathering in accordance with the teams needs and to record these on the patient records.
  • To attend MDT meetings as required for MAS (Memory Assessment Service).
  • To support team lone working ensuring that all staff are safe and accounted for at the end of the shifts.
  • To provide cover and support for patients who are in receipt of care from the four teams in the care coordinators absence if required.
