About the Community Rehabilitation Team:

The Community Rehabilitation Team is a newly formed service delivering evidence-based interventions to promote recovery and support to clients with complex needs across South West London.

The Community Rehabilitation Team will support people in their recovery journey to progress to the next stage of their individual goals and hopes, working with a wide range of colleagues and stakeholders to support functional, active and socially inclusive support.

Main duties of the job

We are looking for a Mental Health Practitioner from one of the core professions with Approved Clinician status to provide highly specialised clinical care within the Community Rehabilitation Team, using advanced clinical knowledge that is evidenced based and within their specific professional skill set.

They will be a very senior clinician with a core professional qualification such as a Nurse, Occupational Therapist, Social Worker or Consultant Psychologist.

This role will function independently and within the Community Rehabilitation team, working alongside and complimentary to the doctor's role in the delivery of health care.

By providing expert advice to ensure the needs of the patient are met, the post holder will lead and enact best practice within the clinical setting.

This is an excellent opportunity for a dynamic and highly skilled mental health practitioner to join a highly specialised new team and lead on the development of rehabilitation pathways and the delivery of optimal services for patients.

It is a requirement of this role to maintain approval to act in the role of Responsible Clinician, as defined in the Mental Health Act 1983 revision 2007, for a specific caseload of patients under the Community Rehabilitation Team and to hold accountability for the delivery of care for clinical decisions made.

Essential

  • oAssessed experience of working as a qualified clinical professional for a minimum of six years, including at least 3 years post qualification experience within the designated specialty where the post is located (Community Rehabilitation Team)
  • oExperience of working with a wide variety of client groups, across the whole life course and presenting with the full range of clinical severity across the full range of care settings including outpatient, community, primary care, in-patient and residential care settings including maintaining a high degree of professionalism in the face of highly emotive and distressing problems, verbal abuse and the threat of physical abuse
  • oExperience of exercising full clinical responsibility for clients' psychological care and treatment, both as a professionally qualified care co-ordinator and also within the context of a multi-disciplinary care plan
  • oExperience of teaching, training and/or professional and clinical supervision
  • oExperience of supervised provision of evidence-based interventions for people with complex and enduring serious mental health problems
  • oExperience of teaching, training and professional and clinical supervision
  • oMust have minimum 6 years post qualification practice at senior level of profession e.g. Post-qualification training in provision of evidence based psychological therapies relevant to the treatment of mental health complex needs
