Specialty Doctor, Forensic Outreach Service (FOS)

Main duties of the job

The Forensic Outreach Service (FOS) is a specialist, multi-disciplinary service operating within the Forensic Mental Health Services to facilitate and manage the safe and efficient transition of high risk adults with mental disorders from secure forensic settings (medium and low secure care) into the community, and eventually, back into their appropriate Community Mental Health Team or Community Forensic Team.

Transition from secure services to the community is recognised as a particularly difficult time. FOS provides ongoing mental health assessment, formulation and treatment to promote recovery and manage risk during and after transition between services in order to safely facilitate patient's eventual acceptance back to their local teams, if appropriate.

Job description

Job responsibilities

The post holder will be based at the Shaftsbury Clinic and will be employed for 5 PAs per week. It would be beneficial for the postholder to have approval under Section 12 of the Mental Health Act, but this can be facilitated during post. The postholder will be responsible for the day-to day management of the patients currently under the care of the Forensic Outreach Service, and activities will include:

  • Attending 2 zoning meetings each week.
  • Take part in assessment of referrals to the service.
  • Assessment and treatment of patients attending Springfield Hospital or in the Community either on their own or with the care coordinator.
  • Liaise with community teams.
  • Attend CPAs and document the outcome of CPAs.
  • Liaise with the patients GP and medical specialists regarding physical health concerns.
  • Prepare reports for Mental Health Review Tribunals and Managers Hearings and attend and give evidence at these.
  • Liaise with families and carers.
  • Liaise with the Ministry of Justice and allied Criminal Justice Services
