Job summary

We are delighted to have secured additional funding for a full time (37.5 hours per week) substantive Community Nurse Practitioner post. This is so that we can continue to implement the FREED approach to enable rapid access to treatment for young adults presenting with the first episode of an eating disorder. We are seeking an experienced enthusiastic qualified nurse to work in our friendly service and focus on the implementation and expansion of the FREED pathway.

We are looking for someone who is eager and passionate about working with a range of clients in the 18-25 age range (some of whom are difficult to engage). The role involves managing all young adult referrals and some transition liaison with our colleagues in the Child & Adolescent ED service. The post holder will join the 'FREED mini team' to track all referrals for 18- to 25-year-olds and conduct an 'engagement call' within 48 hours of a referral being received.

The postholder will endeavour to maintain the enthusiasm for FREED within the team and beyond (building links with relevant services and universities), through training and regular updates/liaison and closely work with the FREED champion to provide assessment and evidence-based treatment for these FREED patients.

Main duties of the job

The role also requires the postholder to take supervised clinical responsibility for a discrete caseload of clients suffering from eating disorders and to provide specialist clinical nursing care to clients under the supervision of the Team Leader and FREED Champion. This will include conducting new patient assessments into the service, acting as named nurse for clients under CPA, being instrumental in involving clients in planning and evaluating their treatment and liaising with other services or professionals as necessary.

The postholder will need to co-ordinating the effective transfer of in-patients to the community team, undertaking group work as appropriate, maintaining excellent documentation/record keeping and correspondence. The postholder will be required to work as part of the MDT team to achieve the highest possible care for the client group, to liaise with clients' families and carers as appropriate and to carefully plan discharges and aftercare as necessary.

It is envisaged that the future ways of working will be agile working, with 2 days in the office and 3 days working from home (subject to change and service demand). CORPORATE ROLES ONLY
