Career Progression pathways and development opportunities:

The aim is to support all Band 5s whether you are Newly Qualified Nurse, A Band 5 with no community experience or are a Band 5 nurse with community experience but want to develop a bit further before stepping into a Band 6 position. We aim to develop your skills and knowledge to be ready to apply for a band 6 nurse role in 12 -18 months.

Main duties of the job

As a Community Mental Health Nurse you will provide community patient led services and work with adults 18-65 years who have mental health issues, social needs and require support with their physical health needs by participating and leading within our health and well being clinics and visiting patients in the community to support their safety and recovery.

Our community multidisciplinary teams, comprise of nurses from Nursing Associates to Advanced Nurse Practitioners, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, mental health practitioners, recovery support workers and peer support workers. The teams are supported by good administrative systems and offers excellent working environments as we transform our older buildings into new with an ethos of team support and team ownership with supportive team managers and community leadership.

Job responsibilities

As a Registered Nurse working in the community you will work under the supervision of a Band 6 Registered Mental Health Professional to promote and deliver evidence-based care for patients with mental health conditions in their own homes and within a variety of accommodation settings.

Community roles vary and the specifics of the community setting in which you will work are covered during induction and will be explained by your line manager and colleagues;

  1. Managing case load, having indepth knowledge and understanding of the needs and recovery goals of each patient. Providing medication, advice and guidance, offering physical health assessments and health promotion. Providing range of mental psychological interventions. Having significant knowledge, awareness and understanding of Mental Health Law, including legislation of particular relevance to the community sector including the Community Care Act (2015) treatment legislation, the Care Programme Approach (2008), the Mental Capacity Act (2005) and Community Treatment Orders (CTO).
  2. Managing own work load as part of the community team (this will be a reduced/protected case load).
  3. Assessing, treating, providing a range of interventions, Co-ordinating care, communicating with other professionals involved in the care and ensuring regular CPA reviews are held as required. This will include communication with other teams for example in-patient services through attendance at ward meetings and with interface services e.g. Addictions service and CAMHS services where appropriate.
  4. Independently carrying out assessment, care planning and care coordination with excellent interpersonal skills, ability to listen to others views, respect and value individuals from a diverse range of backgrounds.
  5. Meeting with community team members on a regular basis to discuss report and evaluating client care.
