Job summary
We are looking for a motivated and proactive individual to join our busy team who is able to demonstrate a high degree of autonomy in assessing patient's mental health care needs, and who can deliver a caring and safe service in a timely and organized way. This is a friendly and supportive team.
Main duties of the job
- To assess/ triage/ act as first entry point for adults into mental health secondary care
- To assess and work with mental health service users and signpost them to other services and discharge to primary care where necessary. Any service users needing longer term secondary care and care coordination will be transferred to the relevant teams in the
- The post holder is expected to utilise community resources, working closely with Primary
- The post holder will ensure user and carer involvement in all aspects of service delivery.
- The post holder will be expected to promote, by example and leadership, a high standard of patient
- To regularly supervise nursing students on practice
- The post holder will communicate and collaborate effectively with all agencies and agents involved in supporting adults with severe mental health

About the team:
Wandsworth Single Point of Access Team (WSPA) provides a borough-based assessment only service for adults aged 18-74, whereby all referrals are channelled through our team, and directed to receive appropriate care either within the Trust or in Primary care. We work closely with the community recovery and support teams, early intervention, crisis resolution & home treatment teams and the AMHP service. We work in close partnership with GP commissioners and other key partners, including voluntary organisations.
About the location:
Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK. This is easily accessible via the Northern Line (Tooting Bec Station) and via private transport. The Trust provides subsidised parking fares for all our substantive staff. Our main site is located immediately next to Tooting High Street where staff can easily access a range of shops, cafs, restaurants etc.
