Learning Disability & Autism Inclusion Lead (Primary Care & Community)

Job summary
As part of the Homerton Learning Disability and Autism Service, the post holder will be an enthusiastic advocate and champion of the needs of learning disabled and autistic people. They will lead on specific projects and workstreams focused on improving health and social outcomes for neurodivergent people (including those with learning disability and/or autism) across City and Hackney. They will work alongside partners across City and Hackney to drive and implement service change and transformation.
They will need to (a) have a specialist level of expertise and understanding of the needs of learning disabled and Autistic adults and children and their carers; (b) be highly skilled and able to make decisions that change patient care pathways and support reasonable adjustments (c) have a good understanding of community services, primary care and related structures; (d) have high visibility and availability within community settings across City & Hackney; (e) have a creative and innovative approach, ability to work independently and identify and set priorities as part of a programme of work

Main duties of the job
Overall main responsibilities :
Support the Trust and the City and Hackney Place Based Partnership in ensuring the following outcomes:
? Improving and increasing the accurate recording of LD and Autism on primary care systems
? Improving uptake and quality of Annual Health Checks and health action plans in primary care.
? Increasing uptake of vaccinations in people with LD and Autism
? Increase the uptake of health screening in the learning disabled and autistic population e.g. cancer screening and wider health screening linked to causes of premature mortality in patients with a Learning Disability.
? Assisting services in primary and community care to make reasonable adjustments, improve accessibility and encourage uptake of healthcare services and amongst people with LD and autistic people.
? Identification of local health inequalities and assist in identifying methods to help address these.
? Work with the relevant stakeholders and experts across the local system to assist in developing health promotion and prevention campaigns and advice relating to risk factors for premature mortality and morbidity in the LD and Autistic population
? Assisting with dissemination of learning from the Leder (learning from lives & deaths) programme and implementation of associated recommendations
