Health Promotion Outreach Nurse

Job summary

The post holder will work in close relationship with the Looked After Children Nurses and follow up any actions identified in Initial and Review Health assessments that require health promotion support or education. Using innovative methods to address unmet health needs among looked after children, with a focus on physical, emotional and sexual health needs, the post holder will deliver behavioural advice to children, young people and care leavers on a one to one and / or group basis. In addition, the post holder will facilitate the Health Improvement Agenda for Looked after children and support delivery of all activities to promote health and wellbeing of Looked after children and care leavers. The post holder must be able to prioritise their activities and therefore improve their time / work management. Some autonomy is given within the role and the post holder will be required to work alongside and support the Looked After Children Health to enhance the delivery of the service, improve the outcomes of looked after children and tackle the wider determinants of health to reduce inequalities for this cohort.

You will need to be an approachable, flexible and committed individual who enjoys developing creative solutions, as well as being an exceptional communicator with the ability to work positively and collaboratively.

Main duties of the job

  • Carry out specialist one-to-one interventions for behaviour change
  • Follow up any health promotion actions from Initial and Review Health assessments under clinical supervision of a LAC Nurse with specific focus on dental health and immunisations
  • Carry out specialist group interventions for behaviour change.
  • Liaise with partners to help reach target populations and communities.
  • Actively seek out 'Hard to Reach' populations and help them to engage in health lifestyle initiatives.
  • Contribute at network meetings and team events.
  • Monitor and evaluate outcomes via processes, procedures, and systems.
  • Give feedback on performance and local intelligence to senior management team.
  • Ensure that all stakeholders receive up-to-date information on a wide range of health improvement topics.
  • Develop, promote and co-ordinate health activities and other community engagement events to deliver health promotion and brief interventions
  • Develop fact sheets and resources.
  • Act as a Health Champion, Making Every Contact Count.
  • Be up to date with a range of health improvement topics through competency-based learning
