Job summary

The Integrated Independence Team have secured new funding for a fixed term contract Band 7 occupational therapist to join our Discharge to Assess service who support people following discharge from out of area hospitals returning home to Hackney and the City of London.

This post is an exciting opportunity for a dynamic occupational therapist able to provide assessment and rapid review of people in their homes to review functional ability, ensure their safety at home and prevent re-admission to hospital.

The role includes anticipatory oversight of need, scrutiny of discharge arrangements and promotion of single-handed care as part of the Discharge to Assess process as well as liaison with a range of services covering the 8 community Neighbourhoods.

You will be part of an established integrated health and social care team and provide specialist occupational therapy input to the multidisciplinary screening of referrals including assessment and signposting or allocation to pathways within the team.

Main duties of the job

The ideal candidate will have strong experience as an acute or community occupational therapist and will be familiar with the therapy and care needs of people leaving hospital and returning to their own homes.

You should have excellent communication skills, a proactive approach, prioritisation and problem solving skills.

You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from senior occupational therapy staff and experienced multi disciplinary staff across the team.

Supervision and in service training are carried out on a regular basis and you will have an annual PDR. There is good support for personal development and opportunities to develop the service in line with best practice.

You will have the opportunity to support and supervise junior staff and students within the service, and to support service development and projects with the team.
