Job summary

We are looking for a Band 6 LAC Nurse for to join our team within the Community Children's Services division.

  • The post-holder will work and support children who are Looked After - LAC and Care Leavers, and their families/ carers liaising with health / external agencies as required, to reach their full potential by accessing the best possible health outcomes.
  • The post-holder will provide holistic health interventions to LAC and Care Leavers
  • The post-holder will work in partnership with the Named Nurse and Named Dr's for LAC to implement National Guidance and Local Policy for Looked after Children
  • The post-holder will work with colleagues to ensure Looked After Children receive timely, child & young person focused assessments and interventions in line with Statutory Guidance.
  • The post-holder will support the Named and Specialist LAC Nurse in ensuring Looked After Children receive appropriate assessments and health provision when they are placed Out or In borough

Main duties of the job

  • To work in partnership with the Named Doctor and Named nurse to undertake holistic health assessments for all children entering care
  • In partnership with the Named Doctor, Named nurse and GP's, develop individual Health Care Plans to ensure that identified or ongoing health needs are being addressed
  • Liaise with all relevant health care professionals in order to implement the Health Care Plan
  • Responsible for undertaking the statutory review health assessments for LAC aged 5 -19yrs
  • Develop, implement and evaluate health care plans in partnership with LAC and their carer's
  • Liaise with all relevant health care professionals in order to implement the Health Care Plan
  • Refer LAC and Care Leaver's onto specialist services as required based on assessment of need
  • To review and evaluate all health care plans for LAC annually or more frequently if required
  • To improve the quality and co-ordination of the health care of LAC and Care Leavers
  • To work in partnership with carers and key workers to enable them to have a good understanding of the health care needs of LAC and care leavers
  • Empower individuals, their carers and families to influence and access services, and act as advocate where appropriate

  • To provide ongoing advice and support to residential care staff with regards to the health needs of LAC and Care leavers
  • To consult with and evaluate LAC and Care Leavers experiences of health services, and develop services that are responsive to their needs
  • Work within the professional boundaries of confidentiality and consent, balancing the need to share information and safeguard LAC and Care Leavers.
  • Ensure all LAC and Care Leavers are provided with a comprehensive health summary upon leaving care.
  • To maintain full, accurate and contemporaneous electronic health records in line with the Trust Record Management Policy
  • To be responsible for undertaking and co-ordinating health reviews for LAC, using agreed health assessment tools in order to improve LAC health outcomes & access
  • Working together with CYP to explore the best ways to engage them and meet their health needs, provided within the context of the core values of the Trust for compassionate care and respect for all
  • Support the effective implementation of transition protocols for LAC and those leaving care, moving from childhood through to adulthood
  • Provide telephone/helpline advice to children and young people, those with parental responsibility, foster carers and residential social workers; acknowledging consent issues appropriate within local and national legislation or guidance.
  • Provide drop-in sessions to children and young people as appropriate & contribute to multi professional support days
  • Provide a liaison role that offers sign posting, child specific liaison and support to access other services, such as Child and Adolescent Mental Health Services, CAMHS-LAC team, Substance abuse, Dental and Integrated Sexual Health Services (ISHS) and the CSE Nurse Specialist.
  • Ensure LAC CYP are given opportunities to have time alone to express any concerns or personal issues to support their health, safeguarding & social care needs; particularly during health assessments
  • To play a key role in increasing the immunisation uptake for Looked after Children
  • To travel to other boroughs as required to ensure effective assessment and implementation of health care plans
  • To use RiO in case management of children and to maintain an overview of the caseload
