Job summary
Are you an experienced Occupational Therapist who feels passionate about improving quality of life for older adults and collaborative multidisciplinary working?
The Enhanced Practice in Care Homes (EPiC) Team is a new multidisciplinary team, who will work closely within the four nursing homes in Hackney to provide early multidisciplinary assessment, short term intervention and collaborative care and support planning to maximise the safety, independence, and quality of life of residents.
Following a 6 month pilot, we have received substantial Ageing Well funding to expand the project to incorporate all nursing homes in Hackney. The Team will provide assessment and short term intervention to support new residents transition into the nursing homes, provide advice and training regarding complex manual handling, posture, positioning, equipment provision, maximising function and promoting meaningful activity for all new residents. The Team will also provide reviews, interim advice and support to current residents.
You will have substantial experience and occupational therapy skills in working with older adults, experience of community working and working in care home settings. You will have excellent communication skills, love working as a team and be motivated to provide leadership.
Main duties of the job
The Adult Community Rehabilitation Team (ACRT) is a forward thinking service that provides an interdisciplinary rehabilitation service to clients aged 16 onwards. The team is split into two specialist branches, physical conditions and neurological conditions. The team consists of physiotherapy, occupational therapy, speech & language therapy, clinical psychology and rehabilitation assistants.
The EPIC team will collaborate with care home colleagues to develop personalized holistic care and support plans, and to facilitate a timely discussion of all residents in a regular multi-disciplinary meeting. The capacity, culture and operational processes of each nursing home will vary and may therefore require different engagement and approach.
The B7 Highly Specialist Occupational Therapist will work across the borough, and with the steering group lead the project in 2 nursing homes, working alongside a physiotherapist (who will lead in the other 2 homes) and a speech and language therapist, the care home teams, dementia nurse and relevant community services to establish early access to therapy assessment, develop a holistic multi- disciplinary meeting and relevant training and support to the care home teams to maximise function, meaningful activity, quality of life and safety.
The postholder will have access to clinical supervision, CPD and support from wider Homerton therapies and ACRT.
