The Team will provide assessment and short term intervention to support new residents transition into the nursing homes, provide advice and training regarding complex manual handling, posture, positioning, equipment provision, maximising function and promoting meaningful activity for all new residents. The team consists of physiotherapy, occupational therapy, speech & language therapy, clinical psychology and rehabilitation assistants.

The EPIC team will collaborate with care home colleagues to develop personalized holistic care and support plans, and to facilitate a timely discussion of all residents in a regular multi-disciplinary meeting. The capacity, culture and operational processes of each nursing home will vary and may therefore require different engagement and approach.

The B7 Highly Specialist Occupational Therapist will work across the borough, and with the steering group lead the project in 2 nursing homes, working alongside a physiotherapist (who will lead in the other 2 homes) and a speech and language therapist, the care home teams, dementia nurse and relevant community services to establish early access to therapy assessment, develop a holistic multi- disciplinary meeting and relevant training and support to the care home teams to maximise function, meaningful activity, quality of life and safety.
