Job summary

The Trust is seeking to appoint a highly motivated and innovative nurse to lead an outstanding multidisciplinary team providing acute and community diabetes care to the people of City and Hackney.

As a Senior Nurse the postholder will be a champion for diabetes care across the Trust.

The postholder will lead a team of diabetes specialist nurses providing care based in GP surgeries and the hospital, in addition to leading teams providing support to people living with diabetes through patient education and specialist type 1 diabetes services.

The post-holder will have clinical experience of managing diabetes as well as excellent negotiating and communication skills.

Informal visits are encouraged.

Main duties of the job

The postholder is responsible, as part of a multidisciplinary clinical team, for delivering specialist nursing care to patients with diabetes within the hospital and throughout the community of City and Hackney.

The postholder will ensure that patients receive the necessary care and treatment to enable progress along an agreed pathway of care in line with the local guidelines and the National Service Framework for long term conditions. This will be achieved through the application of specialist knowledge, skills and practice.
