Job summary

The Inpatient Palliative Care Team at Imperial College Healthcare NHS Trust is expanding to provide a seven-day, face-to-face service.

We are pleased to offer an exciting opportunity for two Band 7 Palliative Care Clinical Nurse Specialists (CNS) to join our team.

As a Specialist Palliative Care CNS, you will provide expert nursing advice and support to deliver high-quality, individualised care for patients and their families or carers. You will work collaboratively with the multidisciplinary team, ensuring care is aligned with local and national standards.

We welcome applications from experienced Palliative Care Clinical Nurse Specialists, as well as motivated nurses with a strong interest in developing their career in palliative care.

The successful candidates will be supported to manage their own caseload, with guidance from senior colleagues, as part of the multidisciplinary Specialist Palliative Care team. You will maintain effective communication with patients, families, carers, and healthcare professionals to ensure a seamless and coordinated service.

In this role, you will also be supported to undertake comprehensive palliative care assessments, contributing to clinical decision-making and prioritisation of care. This includes identifying and addressing patients' physical, psychological, social, cultural, and spiritual needs to ensure the delivery of holistic, high-quality nursing care.

Main duties of the job

  • As a key member of a multi professional team, ensure appropriate referral anddiscussion with other members of the team to optimise patient care.
  • Ensure patients are assessed and have care plans that are implemented and evaluated in accordance with Trust's evidence-based guidelines and protocols.
  • To have open and transparent communication channels that support themultidisciplinary team to do the right thing for people we care for and to speak up when things do not go as planned.
  • Act as a clinical expert for palliative and end of life care.
  • Ensure staffing is in line with Trust policy.
  • Manage resources efficiently and effectively.
  • Champion a culture of learning and quality improvement within the team.
  • Be an ambassador for palliative and end of life care across the Trust.
