Job summary
An exciting opportunity has arisen for a clinical nurse specialist to join the evolving Frailty Team. The Frailty Team currently provide an in-reach service into the Emergency Departments (ED) at both our StMary's and Charing Cross hospitals, assessing patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance.
The service is looking to expand its service byproviding rapid access clinics for complex case management and admission avoidance.
The successful applicant will have experience working with frail patients, ideally within specific frailty services. The role will involve specialist assessments to expertly meet the needs of this complex cohortof patients. Most importantly, the successful applicant will have a passion for older people's care.
Please note that this position requires the successful applicant to be able to cover both the St Mary'sand Charing Cross sites. Currently the service does not operate at weekends and bank holidays.This issubject to change as the service expands.
Main duties of the job
- Working as an integral member of the multidisciplinary team (MDT), the post holder will offer expertnursing care and specialist advice, particularly in relation to frailty syndromes, ensuring high-qualityand individualised care for patients, their families, and carers.
- They will lead on comprehensive geriatric assessments (CGA) for appropriate patients, obtaining relevant social and medical historiesfrom patients, families, care providers and 24-hour care facilities.
- They will exercise professional accountability in all aspects of frailty care delivery, workingindependently within scope of practice and collaborating effectively with the wider MDT.
- They will support patient flow through early intervention and identifying patients who are suitable for admissions avoidance, as well as those who would benefit from an admission with earlyidentification of their needs.
- They will also be actively involved in staff education and development, including utilisingappropriate learning opportunities and to act as a mentor and resource for student nurses,adaptation and unqualified members of the nursing team.
