Job summary
We are looking for a dynamic, organised and highly motivated individual to join our cardiac pharmacy team on our Hammersmith hospital site. The team forms part of a world-class tertiary referral cardiac centre led by specialists with academic interests and leaders in their field. We use cutting-edge clinical innovations to treat and prevent heart conditions and offer a full range of investigations, treatments and management for cardiac conditions.
This role will be focused primarily on leading and developing the pharmacy service provision across the cardiac directorate, and will include an active prescribing role in both an inpatient setting and in pharmacist-led clinics. It is a challenging and varied service, from the cut and thrust of our heart attack centre, through to our cardiothoracic surgery unit, recently HSJ award winning heart failure team, and specialist pulmonary hypertension, inherited cardiac conditions and lipid services.
The post holder must be comfortable working in a multidisciplinary team, have excellent communication skills, show high attention to detail and be able to work under pressure to meet tight deadlines. They must have previous experience of working in the NHS and clinical experience in specialist cardiac services. Most importantly, they must be passionate about delivery high quality patient care.
Main duties of the job
- To provide clinical pharmacy services to patients with cardiac disease in accordance with priorities to ensure safe, clinically effective and cost efficient use of medicines.
- To contribute to the management of drug expenditure through regular analysis and reporting, in order to ensure safe, clinically effective and cost efficient use of drugs.
- To ensure the provision of an effective and efficient service to patients, nursing and medical staff on the cardiovascular wards.
- To participate in consultant ward rounds, multi-disciplinary team meetings and outpatient clinics as required
- To develop, implement, monitor and evaluate protocols and guidelines.
- To ensure that all healthcare professionals are aware of all policies and procedures relating to the service
- To lead on projects relating to the development and implementation of new services, policies and products within cardiology services
- To lead and develop in the pharmaceutical aspects of clinical audit, service evaluation and quality improvement to ensure optimal medication usage
- To work with the medical and nursing staff to develop the role of the pharmacist in outpatient clinics, virtual wards and other models of patient support.
