Job summary
As a registered nurse you will work closely with both the remote monitoring hub and the heart failure team, including consultants, nurses and allied health professional, to identifty and manage patients suitable for admission to our heart failure virual ward (HF VW).
Whether for admission avoidance purposes or for early supported discharge you will provide person centred care to all patients admitted to our HF VW. You will oversee the smooth running of the heart failure virtual ward and collect all necessary data to review and continually improve the service.
A specialist interest both in heart failure and digital technology will be an advantage for this realatively new and exciting post within our already well established multi disciplinary heart failure team.
Main duties of the job
You will have oversight of all patients admitted to our HF VW and work closely with the remote monitoring hub. Specific responsibities will include- Reviewing monitoring data on the remote monitoring platform as requested by the remote monitoring hub.- Ensure all clinical alerts identified on the remote monitoring platform are dealt with in a timely fashion.- Contact HF VW patients as required for medication changes.- Arrange and review blood tests as required for patients on the HF VW.- Work with the wider HF team to action prescription changes and to communicate these with other HCPs/patients.- Review patients in 'same day emergency care' setting (SDEC) as required.- Attend VW MDT and daily Huddles.- Perform home visits as required.- Work closely with other HF and community teams for admission avoidance.- Review BNP biomarker list to identify high risk patient who are inpatients or have been seen by A and E and recently discharged who may be appropriate for admission to our HF VW.

- Evidence of Postgraduate study in Cardiology
- Teaching and assessing qualification or relevant experience
- Knowledge of research and audit including implementation of findings and review of outcomes
Desirable
- Heart Failure Course L7
- Clinical assessment skills training
- Non-Medical prescriber or commitment to undertake
- Working towards Masters Degree/ evidence of working at this level
SKILLS / ABILITIES

- Able to demonstrate highly developed specialist knowledge within heart failure
- Knowledge of heart failure management both within primary and secondary care
- Has a wealth of insight into all aspects of heart failure
- Knowledge of relevant national heart failure guidelines.
