Job summary
The North West London Virtual Hospital (NWLVH) is recruiting experienced band 6 practitioners to provide virtual care and monitoring to patients. The team are based at St Charles Hospital in Ladbroke Grove. Patients remain at home and care is delivered over the telephone and via digital applications. Individuals should have solid clinical knowledge as a registered nurse or AHP, although any specialist clinical background is welcome. No specific IT or remote monitoring experience is required training will be provided but an aptitude for technology is an advantage.
Main duties of the job
The NWLVH undertakes remote monitoring of a variety of patients from across all four North West London acute hospitals. We cover a range of pathways, including cardiology, respiratory, IBD, diabetes, maternity and acute medicine.
Currently we have 19 virtual wards live with a range of SOPs that guide our clinical reviews and we are in the process of developing more pathways. Virtual wards as a substitute for inpatient care, as well as more long term remote monitoring pathways, are being established across the country. This is a great opportunity to be part of a move away from traditional models of care, towards a more sustainable future within the NHS.
