Job summary
A rare opportunity has arisen within the Lipids Department for an experienced nurse to undertake lipid activities in collaboration with stroke services.
The post holder will participate in the pilot project of Lipid Optimisation Pathway Following Hyper Acute Stroke (LOPHAS). They will need to assist the further development of the service and be responsible for running their own specialist lipid clinics alongside the lipid and stroke consultants.
We are looking for an enthusiastic and motivated individual who is an excellent communicator, can work independently and autonomously, demonstrate resilience when working under pressure and can embrace teamwork.
Additionally, this post involves working across two sites (Hammersmith Hospital and Charing Cross Hospital) and is currently on a fixed contract of 12 months. Therefore, it might be suitable for an internal candidate on secondment.
Main duties of the job
- To undertake lipid clinic activities in collaboration with stroke services.
- To increase the proportion of acute stroke and Transient ischaemic attack (TIA) patients who are managed consistently with the acute Academic Health Science Network (AHSH) pathway to optimise their lipid lowering management.
- To provide specialist nurse advice and support for dyslipidaemia patients and take responsibility for their on-ongoing clinical management within both an outpatient and inpatient environment.
- To take responsibility for the nurse-led lipids services, including clinics, counselling, organising biochemical and genetic tests, as well as the provision of treatment.
- To deliver a high standard and evidence-based practice in collaboration with the multi-disciplinary team in line with local and national targets.
- To offer professional support, clinical expertise, specialist knowledge and training to all those professionals who are involved in dyslipidaemia patients' care
