Our Therapy department provides opportunity for enthusiastic and highly motivated therapists to expand their skills in admission avoidance, complex discharge planning, and frailty. We pride ourselves in providing a high quality of care for our patients utilising the Comprehensive Geriatric Assessment (CGA) and working along-side our Frailty Clinical Nurse Specialists.

We are seeking a dynamic Therapist who thrives in a fast paced environment. You will have the opportunity to assess and treat a wide range of patient cohorts; frailty, falls, dementia, delirium, musculoskeletal injuries, neurological conditions and other long term conditions.

Our team consists of occupational therapists and physiotherapists; we are an interdisciplinary team encouraging skill sharing to empower therapists to be holistic and autonomous practitioners whilst also appreciating profession specific competencies.

Main duties of the job

  • The role would provide therapy assessment, treatment and interventions to assist with optimal transfers of care and discharge planning including accessing other AHP specialist opinions where required.
  • As a member of the multi-disciplinary team the post holder will hold a key responsibility in the planning, co-ordinating and delivering of the therapy service to patients to facilitate timely discharge and prevent hospital admissions or re-attendance. This post will be working across the A&E department on the Charing Cross Hospital Site, the assessment/decision making areas attached to the A&E and OPRAC (Older Persons Rapid Access Clinic) as required. Together with other senior staff the post holder will be a key resource both to the Occupational therapy / Physiotherapy service and wider multi-disciplinary teams. The client group includes patients presenting with frailty, a varied range of acute and chronic impairments, such as orthopaedic injuries, falls, neurological as well as other medical conditions. The post holder will also support the Acute Medical therapy team to ensure adequate cover across the Acute and Emergency Medicine pathway. As per requirement the post holder will work 37.5 hours across seven days in a week including weekends and bank holidays as per local therapy rotas across Acute and Emergency Medicine. Depending on the service needs, the post holder will be required to rotate to and/or assist on the other sites, and be flexible with their working hours
