We are seeking enthusiastic and motivated professionals to join the Open Access Follow Up (OAFU) Service; a team of 5 Cancer Support Workers (CSWs), line managed and supported by our Macmillan Patient Support Supervisor. This stratified follow up service supports patients to self-manage, reducing hospital attendances and providing better patient experience, while still safely providing regular monitoring for cancer recurrence. Our team are based at the Charing Cross site, working 8am - 4pm, Monday to Friday.
This is a great opportunity to take on a diverse role incorporating direct patient contact and support delivered over the telephone, as well as administrative duties including data management, case coordination, booking appointments and patient tracking.
The ideal candidate will have experience of working with a multidisciplinary team, a good understanding of cancer pathways, advanced communication skills and excellent administrative skills. Previous experience of using Somerset and Cerner systems would be desirable.
Main duties of the job
This is a telephone-based service supporting cancer patients receiving stratified follow up care under Imperial College Healthcare NHS Trust.
The OAFU Service provides stratified follow up for 4 tumour groups; breast, prostate, colorectal and gynaecological cancers. The OAFU team work to process referrals, coordinate and facilitate protocolised care plans, carry out routine follow up calls with patients over the phone, as well as reviewing and communicating results to patients in a timely manner.
Cancer Support Workers are expected to work both autonomously and in collaboration with their clinical and non-clinical colleagues to support patients post-cancer treatment and ensure follow up care is provided in line with protocol. The role involves a wide array of tasks which require strong administrative skills, attention to detail and abilities to prioritise effectively.
Further support is available for complex queries from the supervisor and if the question is of a clinical nature with further input needed from the specialist team, they will book a telephone appointment for the Clinical Nurse Specialist to call back the patient or their relative.
