The Intgrated Cardio-Respiratory Services is a combined service working across hospital and community pathways, providing community services in community and hospital clinics and home-based settings. The service is a consultant led multidisciplinary team providing a range of service including; diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge support, home oxygen review, pulmonary rehabilitation, dyspnoea management and primary care education and support. The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory service across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes. We also work closely with inpatient teams to provide seamless care.

Main duties of the job

To facilitate and manage the provision of high quality nursing care to all adult respiratory patients alongside the consultants, physiotherapists in respiratory medicine in a variety of settings to promote respiratory health within ICHNT and the surrounding community area of Hammersmith and Fulham. The nurse will provide care for patients discharged from hospital with chronic respiratory disease, patients on Home oxygen, stable disease management at home and in clinic. To assess patients in nurse lead respiratory diagnostic clinics. The nurse will provide support and advice to patients to self-manage their respiratory disease. The nurse will work with the team to assess, plan, deliver and evaluate care for patients with variable care needs. To maintain accurate records to support care provision.
