Are you a motivated, dynamic, and enthusiastic registered nurse with significant experience of working with people that are homeless or rough sleeping?
If so, we have an exciting opportunity for you to be involved in the process of re-establishing our specialist Homeless Health team. We have been awarded three years funding to continue the development of the service.
The Homeless Health Service provides enhanced inpatient care, and ensures safe, and sustainable hospital discharges that meets the needs of patient's currently experiencing homelessness.
You will contribute senior clinical knowledge and expertise in this specialist area through your clinical practice and through education and training. You be a role model for inclusion health practice in the organisation. You will line manage the Housing Officers in the team
Main duties of the job
The Homeless Health Team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge - ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.
