Job summary

An exciting opportunity has arisen for an experienced nurse with a passion for supporting patients living with Parkinson's, their families and carers to join our Neurology Service.

The role involves being a main point of contact for patients, their relatives and carers, co-ordinating the management of care and providing support for individuals living with Parkinson's.

You will be expected to manage a specialist patient caseload working with other members of the MDT, coordinating all necessary care for patients who require their medication to be delivered via infusion pumps.

This role will allow you to develop wider specialist knowledge and skills working in close collaboration with the other Neuro Specialist Practitioners within the team.

You will act as an expert resource for other health professionals working with people with Parkinson's and take a lead role in the holistic assessment of patients. You will be actively involved in the continued development of the service and will need to demonstrate leadership and initiative as well as the ability to develop & sustain good working relationships with multi-professional colleagues & peers.

Main duties of the job

  • Ensure a seamless service through collaboration with the multi-professional teams across Imperial College Healthcare NHS Trust and the community providing specialised advice and support to patients, families and health professionals through the use of evidence based knowledge.
  • Develop patient and service driven initiatives and strive to continually improve the MS service and provide high quality care and expert advice for MS patients and their families.
  • Receive referrals for patients via a Neurologist.
  • Ensure national standards are adhered to through collaboration with the multidisciplinary team, managers and neurologists and raise the profile of nursing and research.
  • Utilise information technology in order to collate accurate and timely information

EDUCATION

Essential

  • Registered Nurse.
  • Teaching and Assessing qualification or equivalent teaching experience.
  • Willingness to undertake MSc in Advanced Clinical Practice or Clinical Neuroscience/ Neuroscience Care.

Desirable

  • Leadership qualification
  • Has attained a Degree in management/professional nursing issues.

SKILLS/ ABILITIES

  • Demonstrate commitment through involvement in Trust wide initiatives/ committees
  • Able to demonstrate highly developed specialist knowledge of own patient group
  • Ability to practice within the scope of the NMC Code of Professional Conduct.

EXPERIENCE
