Job summary
An exciting opportunity has arisen within the ICT department of Imperial College Healthcare, for Nursing or AHP's to become involved in a nurse led remote patient monitoring hub.
The remote patient monitoring hub (RPM) undertakes monitoring of patients from across North West London acute hospitals in a single team. We cover a range of pathways, such as COVID, COIPD, Heart Failure, IBD, and general Cardiac. Currently we have 19 virtual wards live with a range of SOP's that guide our clinical reviews.
Main duties of the job
Clinical Responsibilities
- Act as a clinical expert within ones sphere of work
- Facilitating early discharges from the acute sector as appropriate to the Extended Primary Care Team and Rapid Response.
- Carry out planned care for a group of patients without direct supervision, utilising escalation pathways to ensure care is coordinated
- Ensure the assessment, planning, implementation and evaluation of evidence based, individualised patient care, or assist others in undertaking this assessment
- Ensure all staff maintain accurate clinical observations of the patient and act accordingly
- Have responsibility for the correct administration and custody of medicines according to Trust policy
- Monitoring caseloads in conjunction with team members.
Managerial Responsibilities
- Work collaboratively with colleagues from across the organisation
- Ensure that clinical practices comply with Trust policies and procedures
- Communicate effectively at all times and maintain a harmonious working environment
- Assist the Digital Lead Nurse in compiling duty rotas as required
- Assist the Digital Lead Nurse with the monitoring of sickness and absence levels
- As directed, participate in the selection and recruitment of junior staff.
- Participate in nursing audits and clinical benchmarking exercises and suggest any change to improve standards
- Ensure the patients electronic health record is kept up to date with information and clinical reviews undertaken by members of the team.
