We are expanding our heart failure service and have an exciting opportunity for a dynamic and enthusiastic Heart Failure Specialist Nurse to join our growing team at Imperial. We are an established, integrated heart failure service covering 3 acute hospital sites, one of which has a specialist Heart Assessment Centre. As an integrated service we run community heart failure clinics for our local population as well as managing patients in their own homes. We cover a diverse population of patients admitted to our hospitals and living in London. We provide specialist support and education for our colleagues managing patients within both primary and secondary care. We are committed to improving our heart failure service. We run a remote monitoring service for our heart failure patients, and are in the process of establishing our Heart Failure virtual ward. We support daycase diuresis/intravenous iron injections for our heart failure patients.

The service is fortunate to be led by both a committed Heart Failure Consultant Cardiologist and a Heart Failure Nurse Consultant and as well as a Heart Failure Lead Specialist Nurse. We have a specialist Heart Failure pharmacist on the team and we are in the process of recruiting a psychologist for the service.

Main duties of the job

The Heart Failure Specialist Nurse will work with the wider heart failure team to ensure that patients within the population we serve have timely, equitable access to high quality, evidence based heart failure care throughout their entire pathway

The successful candidate will:

  • Follow NICE guidelines for the management of acute and chronic heart failure and support the service in achieving National targets
  • Will be an experienced practitioner confident in managing a caseload of patients in a variety of settings
  • As part of the multidisciplinary heart failure team will help to develop, implement and evaluate a seamless integrated specialist service, ensuring that patients receive the highest standard of evidence based care
  • Demonstrate a high level of expertise within the service, providing specialist advice, education and support to patients and other health care professionals
  • Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets

Essential

  • Registered Nurse
  • ENB 998/ Slice or C&G 730 equivalent experience
  • Evidence of further training to enhance scope of practice
  • ILS

Desirable

  • Post registration relevant qualification
  • Teaching and Assessing qualification or equivalent teaching experience.

SKILLS/ ABILITIES

Essential

  • Has specialist nursing knowledge of Heart Failure Nursing
  • Able to demonstrate sound and competent heart failure nursing knowledge of working within Primary and Secondary care.
