Job summary

An exciting opportunity has arisen to join the team at Chelsea & Westminster Maternity Service (part of Chelsea and Westminster NHS Hospitals) as a Band 6 Midwife for the diabetic service.

The successful post holder will work closely with the multidisciplinary team to provide a high standard of personalised and evidence based care to women with diabetes throughout pregnancy and beyond. Key working relationships will include the obstetric, endocrine and nursing teams alongside other supportive and allied healthcare professionals and organisations.

Key components of the role are the running of both face to face and virtual clinics, diabetes app management (harnessing technology to support the provision of timely care to women), education for both staff and women to support positive pregnancy experiences and optimum health, care pathway navigation, care planning for pregnancy, labour and birth and audit/shared learning and MDT presentations.

Applicants are expected to have experience in working within midwifery.

Main duties of the job

The Band 6 Midwife for diabetes will be an integral part of the service driving forward the diabetes in pregnancy, birth and the postnatal safety initiatives and supporting a culture of individualised care that promotes a safe and positive pregnancy and birth experience.

The post holder will be passionate and experienced in working with women with diabetes, providing direct maternity care to these women in conjunction with the Lead Diabetes Midwife, Obstetricians, Diabetes consultants, Diabetic Specialist Nurses, Dietician and wider multidisciplinary teams (MDT).

Working collaboratively with the individual woman the role will assess, plan, implement and coordinate their maternity care pathway. The role includes regular input on the wards and supporting the inpatient care of women.

The post holder will work autonomously and within a team. This role also incorporates training and education of the healthcare professionals in the management and care of women with Diabetes in Pregnancy.

Responsible for the operational management of the service and ensure staff have adequate support to enable them to focus on their core responsibilities.

The individual will undertake the key functions of the band 6 role, ensuring the women's and birthing people's experience is of the highest possible standard at all times, with particular emphasis on the provision of dignified and compassionate care, which is delivered in a safe and clean environment.

Level of Clinical Responsibility

  • Provide specialist clinical support to for the diabetes in pregnancy service.
  • Support delivery of the education package to enable women to undertake blood glucose monitoring and recording of this with an application such as the GDM-Health app.
  • Organise follow up visits and triaging of women who are on-boarded onto the GDM-health app.
  • Refer as appropriate any women to the multidisciplinary team,
  • Assist with review of the women with diabetes in pregnancy in all inpatient and outpatient clinical areas as required.
  • Any other duties as may be deemed reasonable by the line manager.
  • To ensure the provision of safe, high quality care and treatment for all patients which is delivered within agreed standards, protocols and guidelines.

  • Work at all times according to regulations as described in ICH Good Clinical Practice (GCP), and to the most current guidance relating to research governance and research ethics in the hospital(s).
  • Be highly organised and maintain/develop systems to maximize administrative effectiveness.
  • Be responsible for the acquisition of records and thorough, high quality data entry.
  • Ensure data is entered in appropriate database.
  • Attend team, local and or national meetings as necessary.

Service Development and Improvement

  • Maintain sustainability of the GDm-Health app.
  • Monitor KPIs set by the project protocol and report as directed to the maternity collective improvement group.

Responsibility for handling difficult/emotional situations.

  • The workload will be variable in timing and site, there will be a need to attend specific clinical areas to review women with diabetes in pregnancy. This is likely to include delivery suite, antenatal wards and clinics.
  • Ability to work sensitively with women, including those who are distressed, bereaved or unwell.
  • Be able to use judgements involving complex facts relating to high risk pregnancies which require the analysis, interpretation and comparison of range of options, prior to approaching patients.
  • Plan the care and support of patients according to project protocols, and to monitor responses throughout the project.
